Introduction to the Care Facility

Dear Patient,
Your stay in Intensive Care may leave you with “memory gaps” or confused and distorted recollections. We have therefore decided to write this diary as if it were a narrative.
This way, once you are home, reading it will help you organize your memories and emotions.

Dear Family Member,
Your loved one’s illness is undoubtedly a stressful event for you. You may feel disoriented and have the sensation of being at the mercy of events.
You too can benefit from writing in the diary, recording your emotions, feelings, or simply telling your loved one what is happening outside the hospital in everyday life. Reading the diary will also help you better understand the care activities in which your loved one is participating.

HOW TO USE THE DIARY

WhoFamily members, friends, healthcare professionals…
WhenWhenever you feel the need.
WhatThoughts, observations, news related to everyday life.
How As if speaking directly to the patient.
Using simple language.
Without expressing judgments and respecting the patient’s dignity.
Without mentioning diagnoses and treatments.
Always signing at the end.

After discharge:
The diary is given to the patient or their family members.

The care staff is available for a meeting and joint reading of the contents.

PRIVACY
The care staff is bound by professional confidentiality regarding all contents of the diary. The data and information contained in it are subject to privacy law and will therefore not be disclosed.
